Before your first visit

Call the member number on your insurance card and ask:
• Is Denitra Gaines / Thoughtful Connections in network for my exact plan?
• Are outpatient mental health visits and virtual therapy covered?
• What deductible remains, and does it apply to therapy?
• What copay (fixed fee) or coinsurance (percentage) will I owe per visit?
• Do I need a referral, prior authorization, or have a visit limit?
Write down the representative’s name and call reference number. A benefits quote is not a guarantee of payment.

Employee Assistance Programs (EAP)

Your employer may offer short-term counseling through an EAP. Ask HR or your EAP administrator about eligibility, covered sessions, authorization numbers and expiration dates. Confirm that our office accepts your specific EAP before starting. EAP benefits may be separate from your health insurance.

Out-of-network reimbursement

If we are outside your network, ask your insurer whether your plan reimburses outpatient therapy, what separate deductible applies, and what percentage of the allowed amount it pays. Ask about claim forms, deadlines and telehealth rules. Contact our office about an itemized receipt or superbill. Reimbursement is determined by your plan and is not guaranteed.
